CAMP HI-SIERRA 2011 INFORMATION:

  1. FORMS AND PERMISSION SLIPS:
    • MEDICAL FORMS:
      Please note, medical forms are required for BSA and Duong will be collecting and controlling them in this month of February.  We need Parts A and B for everything now, and not just Camp Hi-Sierra now.

      Part A:
       General Information.  Required for all events. (ALL IMMUNIZATION SHOTS NEED DATES...TETANUS SHOT WITHIN 10 YEARS!!)
      Part B:
       Parental release and information. Required for all events. (DO NOT FORGET PHOTO-COPY OF HEALTH INSURANCE CARD -- BOTH SIDES!!)
      Part C:
      Physical Exam & Doctor's Signature.  Required for events exceeding 72 hours (example: Camp Hi-Sierra).
      Part D:
      NOT REQUIRED, DO NOT NEED

    • FIREARMS - PARENTAL AUTHORIZATION: (required or no gun usage whatsoever...NO EXCEPTIONS!!)
      All scouts who will, or even plan to, use a firearm should fill out and submit this form to me.  My recommendation is sign it even if scout will not do a shooting merit badge so long as parent is comfortable.
